Combined aftercare is a new form of rehabilitation aftercare for people with addiction disorders, currently being evaluated. By offering greater flexibility, it enables more people to access and complete their aftercare treatment.

Two paths after rehabilitation.
Standard aftercare
Most patients receive a recommendation for aftercare following their rehabilitation for addiction disorders. Funded by the Deutsche Rentenversicherung (DRV) or health insurances, it aims to support patients in maintaining their therapy success after returning home. Although participation significantly increases the chances of long-term recovery, less than half of patients who receive a recommendation actually attend the weekly sessions.
Combined aftercare
Combined aftercare offers a more flexible form of aftercare. It pairs bi-weekly in-person sessions with the digital aftercare tool coobi care. Patients gain more flexibility to balance their recovery with work, family, and other responsibilities, while keeping the crucial benefits of in-person contact. The included Garmin smartwatch tracks sleep, stress, and activity. With the patient's consent, data can also be shared with their therapist for personalised support.

10 in-person sessions
10 bi-weekly in-person sessions (group or individual format) at your aftercare centre throughout the 6-month programme.

coobi care app
The coobi care app provides daily support with aftercare-relevant exercises and tools to strengthen your recovery between sessions.

Garmin Vivoactive 5
A rental Garmin Vivoactive 5 smartwatch tracks sleep, stress, and activity. The data syncs to coobi care and can be shared with your therapist for personalised insights.

Combined aftercare is currently being evaluated by Martin-Luther-Universität Halle-Wittenberg as part of a model project funded by DRV Nord. Participating centres can offer their clients the opportunity to join the study and choose between classic or combined aftercare when starting their aftercare.
The aim of the model project is to evaluate the effectiveness and feasibility of combined aftercare, with the goal of expanding the aftercare offer in Germany.
How to participate or recommend a patient.
To participate, patients must meet the following criteria:
- Insured with DRV Nord, DRV Bund, or DRV Braunschweig-Hannover
- 18 years or older
- Receiving aftercare at one of the participating centres
Participants must bring a signed recommendation for combined aftercare from their rehabilitation doctor to the aftercare provider.
